The mission of the Green County Human Resources Department is to recruit, develop and retain a high performing and diverse workforce that fosters a safe and productive work environment for all Green County Employees.  The human resources staff strives to achieve these goals by creating positive employee relations, encouraging equal opportunity, providing quality customer service and facilitating the duties listed below

Workman's Compensation:
     The following forms must be completed immediately following a work injury or illness
    and faxed or sent to the Human Resources office within 24 hours of the injury or illness:

     -  Pleasant View Bloodborne Pathogens Policy and Forms (Completed by Employee)
Pleasant View Sharps Reporting
     -  Pleasant View Return to Work Form (completed by Physician)

Family Medical Leave Act:
     The following forms must be completed by the employee and submitted to the Department of
    Human Resources as soon as is possible and practical, with 30 days advance notice being optimal:
     -  Family Medical Leave Policy
Employee Request Form
Physician Certification Form
     -  Pleasant View Leave of Absence Request (Completed by PV Employees Only)
